To students who wish to enroll as returnee:
Inquire first if there are vacant slots in the program you wish to enroll (email your Year Level Adviser - see list & email add here) Once there are vacant slots, then you may proceed to the following:
Email the Year Level Adviser (cc: Academic Program Coordinator and DEE Chairperson) about your intention to enroll, and take note of the following:
1. Email subject must be in this format:
"APPLICATION: Enroll as Returnee to _____ from _______ (ID No. / FamilyName, FirstName M.I. / Overall GPA)";
2. Attach in your email the following:
a. Letter of Intent
- Address your letter to DEET Chair thru APC; signed by you and your parent(s)/guardian(s)
b. Approved Leave of Absence (LOA), if there is any;
c. Your Program of Study / Evaluation of Grades; and
d. Filled out Returnee's Application Form (have it signed by your Adviser & Dept. Chair), downloadable through the link below:

e. Study Plan (sample template: [DOCX Format] [PDF Format])
4. Wait for response and/or further instructions from our APC.
